LiveWell Teens groups are open to anyone between the ages of 14-18. By filling out the following form, you consent that you are 14-18 years of age.

By filling out the following form, you understand that support groups are led by trained peer facilitators, and that we do not provide clinical or medical advice.

We do not provide emergency mental health support.  If you are in crisis or afraid of hurting yourself (or others), call 1-800-273-8255; text 'TALK' to 741741; or go to the nearest emergency room.​​

Upon clicking SUBMIT below, you will be re-directed to join a LiveWell Teens Support Group on Zoom. 

The LiveWell Teens Peer Support Group is every Wednesday at 7:00pm US/Eastern time.

The LiveWell Foundation does not share your data with any third party company or organization.

You will need to fill out this form for each meeting you wish to attend.
